Inspector was mobilized to assist the client with a RCFA on some valves that had been ordered. These valves had successfully passed pressure test, and FI and packed according to the procedure but failed when they were installed during SIT. Our inspector enabled the client to find the Root cause of the failure. This was then resolved with close coordination with the supplier and the Client.
